Family Tree Clinic
We provide a wide variety of sexual health services and specialized care for LGBTQ individuals through all stages of life, including low-cost STI testing, limited primary care, and FREE HIV testing for all uninsured patients.

Access to mental health care so queer and trans youth may reclaim their lives from oppression in all its forms. RECLAIM is one of Minnesota's only resources where queer and trans youth ages 12-25 can get financially accessible, specialized mental health care.

Pride Institute
Pride Institute is committed to providing LGBTQ+ people (ages 18 or above) with an inclusive and accepting place for recovery through evidence-based treatment for substance abuse, sexual health and mental health.

MN LGBTQ+ Therapists' Network
A grass-roots group of LGBTQIAPGE2S+ mental health professionals who wanted a way to safely network, support and educate each other and the community about issues related to LGBTQIAPGE2S+ mental health and wellness.

Edges Wellness Center
Small group practice of mental health practitioners that serves marginalized communities and provides individual, couples, and family therapy. Also provides gender specialist services across the lifespan and sex therapy.

Cedar Hill
Provides sensitive, knowledgeable, evidence-based therapy to couples, families, and individuals. Training in sexuality and identity along with extensive training in mental health.

Roots Recovery
Treats substance use and mental illness through a social-justice lens. A trauma-informed and community-based home for behavioral health services. Staff is well-trained to provide support to many cultures and specialize in comprehensive care.

LynLake Center for Wellbeing
Diverse team of providers with expertise across disciplines and specialties. Provide trauma-informed, integrative mental health and wellness services.

Helps people of all ages, genders, and cultural backgrounds who have experienced relationship violence, sexual exploitation, homelessness, mental and chemical health challenges, and other trauma. Throughout the Twin Cities, Tubman provides safe shelter, legal services, mental and chemical health counseling, youth programming, and community education, including public information campaigns to provide community members with the information and support they need to get help or give help.

You will meet with highly skilled staff and care coordinators who will help you explore options and answer any questions you may have.

Planned Parenthood North Central States
Provides health care services in sexual and reproductive health including breast/chest exams, STI testing, birth control, hormone therapy, abortion care, morning after pill, PEP/PrEP, and LGBTQ+ care.

University of Minnesota Transgender Health Services
Transgender health care services for all ages at the University of Minnesota’s clinics. Comprehensive health care including hormone therapy, referrals for gender affirmation surgery, psychiatry and mental health services.

The Adult Gender & Sexual Health (AGSH) Clinic
At Hennepin Healthcare downtown is a medical clinic that is designed to create a safe and comfortable environment for anyone in the gender and sexual minority community to seek care. Hennepin Healthcare’s Adult Gender & Sexual Health Clinic isn’t limited to one type of care. We can manage your hormone replacement therapy and/or PrEP, while also managing your high blood pressure and referring you to surgeons and mental healthcare providers. We have the ability to refer you internally to any of the specialties offered at Hennepin Healthcare. Physicians, community health workers, social workers, and mental health specialists work together to provide informed care in a safe environment.

Supplemental Nutrition Assistance Program (SNAP)
Helps Minnesotans with low incomes get the food they need for nutritious and well-balanced meals. This program provides support to help stretch the household food budget.

Avenues for Youth
Programs not only provide safe shelter and housing options for youth, but holistic supports to meet a youth's mental, physical, and life-long goals. ConneQT is a host home program that provides LGBTQI+ youth culturally responsive housing within their community of choice. 


LGBTQ+ older Americans face tremendous challenges when it comes to health care, and we would love to have your help in bringing awareness to this topic.
